The HIV epidemic presents unique challenges to the public health infrastructure, as meeting the needs of those living with and at high risk for HIV is often intertwined with addressing social determinants of health.

Ryan White funding through HRSA and CDC prevention funding come with a myriad of key performance indicators, policies, directives, and expectations that can often overwhelm local and state health departments earnestly seeking to end the epidemic but often lacking the infrastructure and expertise to conform their epidemic response to federal guidelines. ADAP and 340B program income also present additional layers of complexity as states and jurisdictions are faced with questions in terms of appropriate capture and use of program income and pharmaceutical benefits.

Effective response to HIV is predicated on developing innovations in public health implementation, however already overwhelmed local and state public health agencies often do not have the bandwidth to develop innovation.
